| Purpose: | The Memphis Plan is a ministry of the Church Health Center, which is an ecumenical healthcare ministry. The Memphis Plan is a program of primary care and limited hospitalization for low wage, uninsured employees or self-employed individuals. Services are provided on a volunteer basis by physicians in their offices, labs, and hospitals. |
| Eligibility: | An employer must have less than 200 eligible employees to participate in the Plan. An employee is eligible to enroll if he or she:

| Fee: | $45 per month ($10 employer, $35 employee) $25 per month, for each dependent $120 maximum amount per month, per family |

| Additional Information: | The Church Health Center provides a number of additional healthcare services through its on-site Clinic and the Hope and Healing Center. For additional information concerning these services, call 901 272-0003. |
